Name and Contact info  - All guests are also asked to complete screening forms about their recent status with regard to COVID-19 along with names and contact info (either a phone number or email address) at the venue.

Vaccination - Please bring proof of vaccination (such as a COVID-19 Vaccination Record Card, Excelsior pass, etc.) or a negative COVID-19 test result taken within 3 days prior to the day of the concert. Proof of vaccination or a negative result is now mandatory for indoor events including this event.
